Case Manager

We are currently seeking a Case Manager.  The successful candidate will be responsible for providing linkage and advocacy to ensure that clients and families have access to community resources and other organizations that will help achieve treatment goals.

Duties include (but are not limited to)

  • Perform crisis intervention techniques appropriately and assist as needed in filing reports for situations such as domestic violence, child and elder abuse, and suicidal patients. Record client cases thoroughly, documenting all client contacts according to DMH/DHS regulations and clinic standards, while maintaining confidentially of all patient information in compliance with HIPPA regulations.
  • Review service plans and perform necessary follow-up, track quality and quantity of services as well as client status.  Facilitate goal setting with client where appropriate.
  • When required, prepare initial paperwork with client and provide psycho-social assessments.
  • Work effectively as a team member, demonstrating good oral and written communication.

Requirements

  • Bachelors of Social Work (BSW)
  • Must be fluent in Spanish and English.
  • Knowledge of DMH paperwork desirable.
  • Current CPR Certification Required
